Have a look at the attached screen shot. Blank module name appears with the 'Disengage module' icon in the 'Modules Engaged at Service Level' section.

Steps to reproduce:

1. Access admin console and navigate to 'Service & Service Group Management' page
2. Click on 'Version' service
3. Select 'Manage Module Engagements'
